Case Manager _Location: Rockford, IL_ _Schedule: Monday - Friday 8:00 AM - 5:00 PM (Non-exempt Salaried Position)_ _Valid Driver's License & Personal Vehicle Required_

Make a difference every day. At Stepping Stones, we are committed to providing trauma-informed mental healthcare and community support services that promote recovery, resiliency, and independence. We are currently seeking a passionate and organized Case Manager to join our team and serve as an advocate for clients both within the agency and in the community.

Key Responsibilities

Deliver assertive, trauma-informed case management to decrease hospitalizations and increase community integration. Promote illness self-management by teaching adaptive coping and resiliency skills. Assist clients in accessing public entitlements and connecting with community resources. Support and strengthen natural support systems outside of treatment. Monitor medication use and side effects; document self-administration via the Medication Observation Record. Provide transportation to essential appointments and services. Assist clients with budgeting and banking, including performing payee duties. Maintain accurate, timely documentation and client records in compliance with Medicaid and agency standards. Respond appropriately to crises using emergency protocols and the agency's on-call system. Attend team meetings and trainings; follow all agency policies, confidentiality regulations, and ethical guidelines.

Qualifications

Bachelor's degree or five (5) years of experience in mental health or a related social service field. Meets the qualifications for a Mental Health Professional under Medicaid Rule 140. Strong written and verbal communication, organizational, and problem-solving skills. Must have a valid Illinois driver's license, personal vehicle, and auto insurance.

Physical Requirements

Ability to sit ( 3 hours), stand, bend, climb stairs ( 5 hours) Must be comfortable driving agency vans or personal vehicles for client transport. Reasonable accommodations provided for individuals with disabilities.

Work Environment

Moderate noise levels with direct client contact. Staff must remain focused and free from personal distractions during work hours. Compliance with federal and state COVID-19 vaccination mandates required (accommodations available).
