Case Manager

Location: Lakewood, NJ

Responsibilities Provides case management and direct support to consumers both on site and in the community Implements treatment plans, manages cases, and provides transportation and linkage. Ensures the appropriate completion of all related documentation Functions as an essential member of the team, assisting in shift coverage, participating program activities, and communicating with team members to cover all applicable duties If your skills, experience, and qualifications match those in this job overview, do not delay your application. Qualifications BSW in social work, BA/BS in psychology, or a related field Two years of experience in mental health and human services required Valid New Jersey driver’s license Shift Sunday thru Thursday or Tuesday thru Saturday. 3 days shifts, 2 evening shifts Locations Paterson, NJ Westwood, NJ West Milford, NJ Why Join us ?
