This is a role that offers direct clinical patient care. The primary responsibilities include assessing, planning, implementing and coordinating the delivery of direct patient care on a per shift basis. Key Responsibilities Assess patients' physical and emotional conditions to develop effective treatment plans. Develop and implement individualized patient care plans to meet the unique needs of each patient. Collaborate with healthcare teams to ensure seamless delivery of care and achieve optimal patient outcomes. Maintain accurate and detailed records of patient interactions and treatment progress. The ideal candidate for this role will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
