We are seeking a compassionate Registered Nurse to lead our Hospice team in providing exceptional end-of-life care. As a Case Manager, you will be responsible for developing and implementing personalized Plans of Care for patients, ensuring their physical, psychosocial, emotional, and spiritual well-being. This is a PRN/Per Diem position that involves home visits for Hospice. We require experience in Hospice or Home Health settings. Flexible schedule and evening/weekend availability are preferred. The Registered Nurse Case Manager assumes full responsibility for delivering high-quality care to each hospice patient. They continuously evaluate personal and professional performance to increase productivity and quality of care delivered, adhering to and supporting the organization's policies/procedures/goals/objectives to ensure exceptional patient care. Delivers high-quality care to each hospice patient Evaluates personal and professional performance to increase productivity and quality of care Supports the organization's policies/procedures/goals/objectives to ensure exceptional patient care Makes sound critical decisions regarding patient care delivery without compromising quality or cost-effectiveness Develops and implements treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being Requirements One year of clinical experience Licenses and Certifications Current RN licensure in the state of practice and one year of clinical experience Current CPR Certification Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
