Job Title: Healthcare Navigator
About the Role: A Senior Registered Nurse Patient Navigator is responsible for overseeing the development, implementation and management of an integrated medical benefit program. This includes case management and health/disease management throughout the continuum of care. The incumbent will interact with providers and plan members to provide efficient and effective service. Key responsibilities include developing and managing an integrated medical benefit program, coordinating patient care, interacting with healthcare providers and patients, identifying targeted chronic diseases/high risk individuals and developing programs to reduce financial and health risks. The successful candidate will have an Associate's Degree in Nursing and 3-5 years of experience as a Registered Nurse. They must hold an RN License in the state of employment or compact and possess Basic Life Support (BLS) certification.
