Job Title: Home Healthcare Professional This is a summary of the job description, up to 50 words. Job Overview: The Registered Nurse Home Health coordinates patient care activities with other members of the health care team in a collaborative and supportive environment. The nurse will provide direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Key Responsibilities: Developing and implementing patient care plans in collaboration with healthcare professionals. Providing high-quality, evidence-based care that meets the unique needs of each patient. Collaborating with family members and caregivers to ensure seamless care transitions. Required Skills and Qualifications: Education to meet certification, license or registration requirements. Oregon Registered Nurse License upon hire. National Provider BLS - American Heart Association within 30 days of hire. 1 year - Medical/surgical experience in an acute care setting. 2 years - Experience in acute care or community mental health setting for Mental Health RN work assignment. Preferred Qualifications: Recent Home Health experience. Hospice or Palliative Care experience for Palliative Care RN work assignment. Point of Care experience.
