Job Title: Inpatient Wound Care Coordinator This role involves coordinating clinical activities of the Inpatient Wound Care Program, including patient assessment and planning, evaluating patient progress, and providing direct patient care appropriate to the age of patient served. The successful candidate will be responsible for assessing and diagnosing wounds, ostomies, and continence disorders, developing and implementing treatment plans, monitoring and evaluating patient progress, and providing patient education on self-care and disease management. Key responsibilities include: Evaluating patient assessments to determine the need for wound, ostomy, and continence nursing services Developing and implementing treatment plans in collaboration with interdisciplinary teams Providing direct patient care, including wound dressings, ostomy care, and continence management Collaborating with healthcare providers to ensure comprehensive patient care Documenting patient information and treatment plans in accordance with hospital policies Requirements Graduate of a program in discipline Bachelor's degree preferred Current licensure by WV State Board of Nursing One to two years related experience and/or training preferred Wound Care Certification preferred Two years of experience in wound, ostomy and continence care required per Wound Ostomy Continence Nursing Certification Board Why This Role Matters This role plays a critical part in delivering high-quality patient care and improving patient outcomes.
