* Conduct thorough patient evaluations and develop appropriate treatment plans

* Implement strategies to reduce hospital admissions among elderly patients

* Utilize eClinicalWorks electronic health record system for accurate documentation and coding

* Collaborate with interdisciplinary healthcare teams to ensure optimal patient care

* Adhere to all facility policies, procedures, and safety protocols

* Maintain up-to-date knowledge of current geriatric practices and treatments

Additional Information:
