About the Role:
As a Patient Care Coordinator, you will be the essential link between patients and providers, ensuring smooth care delivery from start to finish. You will be responsible for:
Educating and supporting patients throughout their treatment journey by explaining procedures, setting expectations, and providing clear post-care instructions
Managing care coordination logistics, including scheduling consultations, procedures, and surgeries
Handling insurance verifications, authorizations, and payment coordination
Preparing exam rooms, updating and maintaining accurate patient charts, and supporting providers during clinical visits
Building trusting relationships with patients and delivering a concierge-level experience with professionalism and compassion
Who We're Looking For:
We are looking for a dynamic, motivated team player with:
Previous experience in a healthcare or medical office setting
A strong understanding of patient care coordination and workflow
Proficiency with Electronic Medical Records (EMR) systems
Exceptional organizational skills and the ability to multitask in a fast-paced environment
Excellent communication skills and a patient-first attitude
Bilingual fluency in Spanish is a plus

In accordance with the New Jersey Pay Transparency Act, we disclose that this position falls within Pay Grade 4 of the 2025 OPM Salary Table applicable to the NY, NJ, CT, and PA region. Compensation will be based on experience and education.
