Our palliative care programs offer a wide range of services in facilities and communities to support patients and their families with life-limiting illnesses.
Key Responsibilities: Conduct physical and psychosocial assessments for hospice patients, documenting visit details electronically.
Develop personalized nursing plans of care based on patient needs, making recommendations as required.
Communicate with physicians to request written orders for plan changes.
Supervise and train home health aides working with our hospice patients.
This is a field-based role that requires excellent communication, listening, and teaching skills. The ideal candidate will have experience working within an interdisciplinary team environment and be proficient in inputting clinical data.
