Job Title: Patient Care Coordinator About the Role: The Transfer Center plays a vital role in ensuring seamless patient transfers into the healthcare system. The position requires strong collaboration with business partners to determine the appropriate level of care and transport for patients. Key Responsibilities: Gather essential medical and demographic information, facilitate communication between referring and receiving physicians, and coordinate effective transfers between healthcare facilities. Work closely with facility House Supervisors, referring physicians, ED and inpatient staff to prioritize critical transfer issues, escalate concerns when necessary, and maintain timely knowledge of all current and future transfer requests. Skill Requirements: Excellent customer service skills, problem-solving abilities, and proficiency in computer systems, including EMTALA, Meditech, and transfer center software. Maintain up-to-date knowledge of hospital services, special programs, and physician/service changes, as well as establish and maintain awareness of bed availability and service changes. Professional Qualities: A high degree of professionalism, a patient-centered mentality, and the ability to prioritize multiple complex tasks while maintaining accuracy and efficiency.
