May design a drug therapy plan for patient-specific problems and persuasively justifies recommendations.Provides enhanced patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patient’s, providers, and pharmacy.Provides consultation to other health care professionals if requested.For Specialty Pharmacy Only: Conducts patient counseling and monitoring as required by the patient management program.For Specialty Pharmacy Only: Participates in the quality management program to facilitate continuous process improvement initiatives and quality improvement projects.Precept pharmacy students and residents, as applicable.Works in a constant state of alertness and safe manner.Performs other duties as assigned.EDUCATIONPharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacyEXPERIENCENo experience requiredPHYSICAL REQUIREMENTSFrequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Frequent use of hearing and speech to share information through oral communication.
