The Population Health Coordinator is accountable for assuring a high standard of quality and care management support for the value based care contracts (VBC) with which OnPoint participates. This position is responsible for having a full understanding of the quality parameters and payment methodologies associated with the various quality programs. The Population Health Coordinator will be working to optimize performance in VBC contracts through the utilization of Quality Improvement tools, in collaboration with quality payer program representatives to achieve measurable outcomes. This person works in partnership with the Population Health Manager to develop and implement corporate workflows to guide the work of the Population Health team.

ESSENTIAL DUTIES AND RESPONSIBILITIES
DUTIES INCLUDE BUT NOT LIMITED TO:
   Obtain and analyze quality program metrics and reports for care opportunities via payer portals.
   Conduct chart review, data entry, documentation submission and patient outreach as needed for various quality program gap closures.
   Review payer reports and assist management in the interpretation of the actions needed.
   Participate in design, development and implementation of corporate level quality program initiative workflows.
   Communicate and partner with OnPoint Executive Directors of Operations to support practice teams in office based care delivery interventions resulting in cost of care savings and improved health outcomes for patients.
   When the need arises, will perform other duties as assigned by supervisor.

QUALIFICATIONS
Licensure/Certification/Education/Experience
   LPN License or Medical Assistant Certificate preferred
   Experience in primary care, care management/coordination or other related field.
   Commensurate experience in public health, social services or other health care field may take the place of this requirement.
   Competence in the ability to triage patients over the phone and in person;
   Strong computer skills, particularly related to Microsoft applications (Word and Excel)
   Comfort with exploring and learning to use new or unfamiliar applications or databases.
   Working knowledge of health information technology

$25 to $32 Per Hour
