Job Summary and QualificationsThe Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
What qualifications you will need:
Education & Experience:
- Associate Degree in Nursing* or Nursing Diploma Required
- Bachelor’s Degree in Nursing Preferred
- 2+ years experience in case management OR 3+ years experience in clinical nursing Required
- InterQual experience *Required Associate Degree in Nursing for any hires after 05/01/2020. Bachelor’s degree preferred. Preferred
Or equivalent combination of education and/or experience
Licensure, Certifications, Training:
- Currently licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation. FT/PT/PRN colleagues with a compact license from a state other than Texas must:
- apply for a Texas RN license within 60 days from hire
- obtain current Texas RN license within 6 months of application for Texas RN license 60 Days to apply for Texas RN license; 6 Months to obtain TX RN license
- Certification in Case Management Preferred
