As a LTSS Service Care Manager, you will play a critical role in developing and coordinating holistic care management activities to achieve effective healthcare outcomes. This field-based position requires regular in-person visits to member homes or facilities, with some remote work. Ideal candidates reside in Pinellas County, FL. Care Management Activities Evaluate member needs, identify available resources, and recommend optimal plans for best outcomes. Develop ongoing long-term care plans, identifying necessary providers, specialists, and community resources. Coordinate between members/families/caregivers and care provider teams to ensure accessible services. Provide support for employment, housing, participant direction, independent living, justice, foster care, and other needs. Monitor care plans, member status, and outcomes, providing recommendations as needed. Interact with long-term care healthcare providers and partners to meet member needs. Collect, document, and maintain member information and care management activities. Perform home visits to assess member needs and collaborate with healthcare providers and partners. Offer education on procedures, service options, referrals, and healthcare benefits. Provide feedback on opportunities to improve care and service delivery. Requirements A Bachelor's degree and 2-4 years of related experience are required. Alternatively, equivalent experience may be considered.
