Job Summary: We are seeking a highly skilled Senior Registered Nurse Patient Navigator to join our team. This individual will provide health and wellness coaching, case management services, and expertise regarding patient care. They will be responsible for monitoring cases and claims, analyzing biometric and health risk assessment data, organizing disease management programs, and proactively interacting with patients, physicians, and other providers. Key Responsibilities: Develop, implement, and manage integrated medical benefit programs, including case management and health/disease management. Coordinate patient care throughout the continuum of care using standard tools, databases, and methodologies. Interact with providers and plan members in case management medical activities to provide efficient and effective service. Identify targeted chronic diseases/high-risk individuals and develop programs to reduce financial and health risks. Monitor and evaluate the effects of case management on patient populations. Analyze demographics, utilization, and reimbursement data to inform program development. Maintain accurate records and prepare reports to determine the reasonableness of claim/cost data and utilization rates. Collaborate with employer groups to develop benefit structures and make recommendations based on data analysis.
