Outpatient Rehabilitation Services We are seeking a skilled Physical Therapist to provide rehabilitation services in an in-home setting. Key Responsibilities: Evaluate and treat patients in their homes or independent living facilities Develop and implement individualized physical therapy programs to restore, reinforce, and enhance patient performance Instruct and educate patients and caregivers on essential skills for promoting independence and productivity Select and apply appropriate therapeutic techniques, including exercise, manual therapy, and modalities Design, fabricate, fit, and apply adaptive devices as needed Additional Responsibilities: Assess patient needs and develop comprehensive plans of care Implement treatment plans and document progress accurately Maintain accurate records and reports Collaborate with other healthcare professionals to ensure comprehensive patient care Participate in staff meetings and contribute to patient care discussions Benefits of the Position: Opportunity to work with older adults in outpatient rehabilitation settings Challenging and rewarding work environment Collaborative team-based approach to patient care
