Home Health Physical Therapist We're seeking a compassionate and skilled physical therapist to join our home health team. As a home health physical therapist, you'll have the opportunity to work with patients in their own homes, providing one-on-one care and developing personalized treatment plans. Your role will involve assessing patients' physical abilities, creating and implementing treatment plans, and collaborating with other healthcare professionals to ensure seamless care coordination.
