Principle duties of this role is to provide clinical mentorship to ensure plan of care accuracy, utilization management and timely submission of documentation to ensure success in the new Medicare payment structure - Patient Driven Groupings Model (PDGM).
ChristianaCare HomeHealth is seeking a Full Time RN to work in either our Smyrna or Avenue North office providing mentorship to clinicians to ensure timely and accurate plan of care development and maintenance. The successful candidates will work Monday through Friday between 9am-5:30pm; and a weekend rotation - usually 1 out of 7. May have the opportunity after orientation to work the weekend commitment remotely. No holiday requirement.

Duties and Responsibilities:
* Ensuring visit plan and ongoing care is appropriate using principles of Case Management/Utilization Management
* Collaborating with Quality/Education/Clinical Operations regarding performance improvement opportunities
* Providing expert guidance to clinicians regarding accurate completion of OASIS assessment
* Collaborating with medical coding personnel to ensure appropriate documentation and supporting documents are present in the medical record
Requirements:
* DE RN license
* BSN
* OASIS certification required (or must become certified within one year of employment)
* Case Management or Ambulatory Care Certification preferred
* Two years of Home Health experience preferred, may consider hospice or insurance case management experience
