Job Title: Clinical Care Coordinator The successful candidate will work in a dynamic clinical setting as a skilled and compassionate healthcare professional. They will be responsible for providing high-quality patient care, addressing the complex needs of patients and their families, administering medications and treatments, documenting patient information, and assessing learning needs to provide education. Key Responsibilities: Utilize the nursing process, including assessment, diagnosis, planning, intervention, and evaluation, to deliver excellent patient care. Address the psychological, emotional, cultural, and social needs of patients and their families in accordance with their level of practice. Administer medications and treatments effectively, monitoring for side-effects and the effectiveness of prescribed treatments. Document patient history, symptoms, medication, and care provided accurately. Assess learning needs and provide education to patients, family members, and caregivers, identifying issues and resources. Requirements: Bachelor of Science Degree in Nursing, preferred. 1 year of experience in the related nursing specialty, preferred. RN License in the state of employment or compact required. Current ACLS certification required.
