Position Overview

We are seeking a compassionate and skilled Hospice Registered Nurse to deliver high-quality end-of-life care to patients and their families. This role ensures timely, coordinated, and holistic hospice services in alignment with the interdisciplinary care plan and physician directives.

Territory: Pike, Upson, Lamar, Meriwether, Crawford, Talbot,

 Essential Job Functions:

• Conduct regular and after-hours home visits to assess patient/family needs and document care plans, interventions, and outcomes.

• Collaborate with the interdisciplinary team to develop and revise care plans, ensuring continuity across providers and settings.

• Supervise Hospice Aides and LPNs, ensuring care aligns with established plans and documentation standards.

• Participate in on-call rotations, long-term care coordination, and facility care plan meetings.

• Educate patients, families, and caregivers on hospice philosophy and services.

• Maintain compliance with Medicare, Joint Commission, and agency standards.

• Support community outreach and program growth initiatives.

• Manage medical supplies cost-effectively and participate in quality improvement activities.

• Ensure timely, accurate documentation and uphold privacy and security protocols.

Requirements:

• An active RN License in the state of application
• Valid CPR
• Valid drivers licesne and proof of automobile insurance
