Job Title: Patient Care Coordinator
Evaluating, coordinating, processing, and documenting patient transfers into the CHRISTUS Health System.
Collaborating with relevant business partners for appropriate level of care and transport of patients into the health system.
Obtaining medical and demographic information and coordinating communication between healthcare facilities.
Key Responsibilities:
Exhibiting excellent customer service skills, including telephone and personal contact with all groups.
Maintaining a high level of professionalism at all times.
Maintaining a patient-centered mentality at all times.
Anticipating and prioritizing critical transfer issues.
Escalating concerns/barriers that require administrative judgment based on hospital policies and procedures.
Required Skills & Qualifications:
Associate's Degree in Nursing.
RN License in state of employment or compact.
BLS Certification.
