Personal Care Physical Therapist

As a skilled physical therapist, you will be responsible for providing high-quality rehabilitative care and skilled nursing to clients in the comfort of their own homes. You will make home visits to assess clients' physical needs and develop personalized care plans that meet their unique requirements. Responsibilities: Conduct thorough assessments of clients' mobility, strength, and balance to identify areas for improvement. Develop and implement individualized care plans that include manual therapeutic exercises, gait training, and other interventions as needed. Continuously assess and revise client care plans to ensure optimal outcomes and collaborate with nursing staff as required. Educate and instruct clients and their families on necessary skills and strategies for independent living. Qualifications: A current license to practice as a physical therapist in Pennsylvania. A minimum of one year of recent work experience as a physical therapist. Graduation from an accredited program in physical therapy. Strong interpersonal, organizational, and problem-solving skills. Solid computer skills, preferably with experience using electronic medical records (EMR).
