Summary:
Responsible for Diabetes Mellitus (DM) instruction, documentation, and follow-up on patients identified through physician referral. Conducts quality and effective diabetes self-management education program classes. Accountable for ongoing evaluation of program, performance improvements, physician and patient phone calls/correspondence. Primarily responsible for outpatient appointments. Develops, implements, and evaluates community educational events. Participates in community diabetes screenings. Maintains ADA or CDCES Diabetes Recognition Program status.
Responsibilities:
- Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
- Provides quality, evidence-based outpatient diabetes self-management & medical nutrition therapy instruction in group or individual settings based on assessed needs and consistent ADA or ADCES guidelines.
- Instructs according to age-specific guidelines & medical conditions. Applies AADE self-care behaviors to educate on and initiate behavior change.
- Maintains/supports Diabetes Recognition Program status by assisting with ADA or ADCES annual accreditation and related tasks.
- Performs patient DM instruction per physician orders.
- Instructs according to age-specific guidelines & medical conditions.
- Provides family support & education PRN.
- Refers to self-management classes when appropriate.
- Proficient in providing instructions on Diabetes technology-based devices.
- Documents and evaluates the teaching and learning process in the patient’s chart.
- Provides professional and timely info to the MD regarding patient progress/status.
- Maintains/supports ADA or ADCES Diabetes Recognition Program status.
- Develops, implements, and participates in community diabetes and educational programs.
- Evaluates the effectiveness of community outreach events.
- Assists with diabetes education in-service for nursing staff/physicians.
- Participates in multidisciplinary meetings with the Diabetes Advisory Board committee and other healthcare professionals involved in the care of diabetes patients.
- Represents and promotes CHRISTUS Health.
- Provides input and assists with the planning of the Diabetes Center’s services.
- Facilitates participant access to services.
- Strives for the highest standards of productivity and quality instruction.
Job Requirements:
Education/Skills
- Bachelor's Degree in Nursing required
- Master's Degree in a related field is preferred
Experience
- 1 year of experience in the related field is preferred
Licenses, Registrations, or Certifications
- RN licensure in the state of employment is required
- BLS is required
- Certified Diabetes Care and Education Specialist (CDCES) or Board Certified - Advanced Diabetes Management (BC-ADM) is required within 24 months of hire
- Certified Pump Trainer (CPT) is preferred
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame.
