Service Care Coordinator Job Description This role involves developing, assessing and coordinating holistic care management activities to achieve quality and cost-effective healthcare outcomes. We're looking for a skilled Service Care Coordinator who can reside in the Council Bluffs / Omaha area and travel locally 50% of the time. The remaining time can be worked remotely from home. Evaluate members' needs, available resources and recommend personalized service plans Develop ongoing long-term care plans and identify necessary providers, specialists and community resources Coordinate between members and care provider teams to ensure accessible services Provide resource support to members and their families/caregivers Monitor care plans and member outcomes, making recommendations as needed Interact with long-term care healthcare providers and partners to meet member needs Collect, document and maintain member information and care management activities Maintain compliance with state, federal and third-party payer regulators The ideal candidate will have a Bachelor's degree in a human services field and at least one year of experience delivering services to similar population groups or an Iowa license to practice as a registered nurse with three years of experience.
