About the Role: We are seeking a compassionate and skilled Speech-Language Pathologist to join our team in a skilled nursing facility. The SLP will work with adult and geriatric residents to assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders, helping them improve their quality of life and functional independence.
Key Responsibilities: Evaluate residents with speech, language, cognitive, and swallowing disorders Develop and implement individualized treatment plans Provide therapy services in accordance with physician orders and care plans Collaborate with interdisciplinary team members including nursing, OT/PT, and dietary staff Maintain accurate and timely documentation in compliance with facility and regulatory standards Educate residents, caregivers, and staff as appropriate
Qualifications: Master's degree in Speech-Language Pathology from an accredited program Active state SLP license Certificate of Clinical Competence (CCC-SLP) preferred; CFY may be considered SNF or geriatric experience preferred, but not required Strong communication, organizational, and problem-solving skills
