Days, 1:20 patient ratio, every other weekend required, holiday availability required Highlights: Case load up to 25 patients in an acute care facility setting Epic charting system experience required Comprehensive orientation of 40 hours provided Work with interdisciplinary teams including social services, pharmacy, respiratory, and more Business attire or scrubs acceptable Up to 7 days of requested time off available Requirements: Active California RN license required (pending accepted but must be active by start date) Minimum 2 years acute care case management or discharge planning experience Previous travel experience required; first-time travelers considered with appropriate screening InterQual criteria experience highly preferred Expertise in admission criteria, care coordination, and discharge planning Knowledge of Medicare, MediCal, HMOs, and insurance authorization processes Competency with DME arrangements, SNF placement, and ambulance transport coordination Experience serving diverse patient populations including neonates through geriatrics Proficiency with HIPAA guidelines, DRG coding, and CMS regulations Must commit to working 3 of 5 major holidays during assignment Why It's a Great Opportunity: This position offers the chance to make a meaningful impact on patient outcomes through comprehensive discharge planning and care coordination in a well-supported acute care environment.
