    Job Details
  • Responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home.
  • Work in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team members to maximize participant independence and safety and develop or restore function.
  • Collaborate with the Occupational Therapist to efficiently operate WelbeHealth rehabilitation department.
  • Member of the WelbeHealth management team and facilitate collaborative relationships across WelbeHealth and with external stakeholders to promote WelbeHealth values, team culture and mission.
  • Conduct Physical Therapy assessments to determine care needs, preferences and goals of participants.
  • Deliver and document Physical Therapy interventions including active and passive exercise, muscle re-education, gait and functional training of ADLs, and treatments such as massage, heat, sound, water, light or electricity.
  • Supervise Therapy Assistants including providing training, instruction and guidance to implement Functional Maintenance Programs.
  • Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
  • Order, track and manage maintenance for equipment.
  • Remain alert to input from team members, participants, and caregivers and update the interdisciplinary team promptly of any changes in participants’ condition or functional status.
  • Support smooth care transitions between settings such as hospitals, skilled nursing facilities, and home.
  • Support enrollment of prospective participants into the program in coordination with the Marketing Team.
  • Participate in end-of-life care coordination and support.
  • Manage direct reports including training, supervising and mentoring.
  • Be legally authorized to practice in position location and act within scope of authority.
  • Complete all required documentation in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
  • Follow WelbeHealth policies and procedures and participate in required Quality Improvement activities, staff training and meetings.
  • Communicate regularly with Supervisor and team regarding workload and priorities.
  • Timely completion of all mandated trainings and education and occupational health screenings as needed.
  • Exercise flexibility in performing assignments as business needs evolve.
    Job Requirements
  • Five years of progressively responsible Physical Therapy experience.
  • Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population.
  • Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
  • Two years supervisory experience with demonstrated ability to mentor and develop team members.
  • Ability to process accurate information in a timely manner.
  • Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
  • Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
  • Highly motivated, inspirational leader able to innovatively engage, support and remove barriers for the team to meet established goals.
  • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to prioritize in a fast-paced environment.
  • Experience and competency working with people from diverse backgrounds and cultures.
  • Commitment to unlocking the full potential of vulnerable seniors.
