**Responsibilities:** • Assess patients' physical, emotional, and social needs and develop individualized care plans • Communicate effectively with patients, families, and the care team to ensure seamless care coordination • Lead our care team to deliver high-quality patient care and achieve excellent outcomes • Stay up-to-date with the latest medical research and best practices in nursing to enhance patient care

**Qualifications:** • Current RN licensure in good standing • 2+ years of experience in a clinical care setting (hospice experience preferred) • Strong communication and interpersonal skills • Ability to lift at least 25 lbs and bear the weight of an average adult effectively
